Application process

To apply for residence at one of our recovery houses:

1.    Review the Policies and Procedures document.

2.    Fill out the application.  All answers must be completed.

      Download an application and fill it out by hand.   You can fax it in or scan it and email it

3.    If you are taking any medication, we will need a letter from the prescribing physician listing ALL current medications.

4.    Send your application and physican's letter (if appropriate) to the Monarch Recovery staff.  You can :

    5.    Your application will be reviewed and you will be contacted by phone within 24-hrs to set up an interview.

6.    You will have an interview with a member of the Monarch Recovery staff.

7.    Your move-in day can be scheduled once you have approval from the Monarch Recovery staff member and your deposit has been received.


A deposit of $300 is required to hold your place at the recovery house.   The deposit covers your first week’s rent and last week’s rent. 

Acceptable forms of payment are cash, cashier’s check or money order.
